For a complete face to face discussion – we come to you!

This in-home consultation includes:

  • Full review of your current health, function and well-being in your home and community so you can keep living your best life
  • Discussion about what funding and services are available and how you might use these to your best advantage
  • Full explanation of Government-funded home care services (including My Aged Care, Commonwealth Home Support and Home Care packages) – how it works, what the costs are for you and how to best utilise available services.
  • How to get an assessment, what to ask for and how to put the care in place once you have approvals.
Home Care Navigation

If you don’t know where to start – we can take you from wherever you are and move you towards where you want to be. We have extensive experience in asking the right questions to make sure you understand everything and can be a truly informed decision maker when it comes to your needs and goals.

If family or loved ones wish to take part but cannot attend directly, we can arrange for a Zoom link-up so that they can be involved.

A summary report is completed following this assessment and can be sent via email or post. This will summarise our discussion and give you a step-by-step plan to implement the recommendations so you can reference or revisit any of our discussion points whenever you need.

(Note: additional travel charges may apply for areas >20km from Glenelg SA, including Adelaide Hills and Fleurieu Peninsula)

For a more in-depth assessment of your current situation and needs – where you prefer not to meet face-to-face, or cannot due to location. 

As for the In-home Consultation & Planning session – we answer all your questions and thoroughly cover your needs, wants and goals. Then we can send you a summary via email, including an easy step-by-step guide on how to get care and get it in place.

This session will include a follow-up session to ensure any questions are answered once the information has time to settle in.

FOLLOWING THE CONSULTATION AND PLANNING SESSION YOU WILL HAVE A COMPLETE STEP-BY-STEP GUIDE OUTLINING A COMPREHENSIVE PLAN, WHAT TO DO NEXT AND HOW TO GET THINGS IN PLACE

IF YOU WANT THE OPTION OF MORE HELP, WE WILL ALSO PRESENT A SERVICE OUTLINE AND COSTS FOR US TO DO IT ALL FOR YOU.

WE CAN:

ACT AS YOUR ‘MY AGED CARE’ REPRESENTATIVE to get approvals, assessments and support arranged.

  • We can lodge an allied health-based request for an ACAT Assessment, with a thorough report supporting evidence for your care needs.
  • Once the ACAT is completed, we can assist you to put services in place and act aas a point of contact to make sure they are running smoothly in the first month.
  • ARRANGING OTHER SUPPORTS – from telehealth conferences with your GP, to sourcing equipment, to referring for other support services, let us sort it out.

PROVIDER FINDER – Once we have established how you need your package to look and created a baseline budget, we can do all the legwork to gather information from providers.

  • We know the questions to ask, the provider obligations, your rights and what you need your provider to do for you.
  • We then bring this information back to you so that you can make the very best informed choice when putting your care in place.
  • We DO NOT align with any providers – our research and information is independent and directed towards getting the very best outcome for you.

HOME CARE PACKAGE REVIEWS – we conduct independent reviews of existing home care packages

  • Whether you have a self-managed package and have been left in the dark as to how to use your funds, or are not sure if you are getting the correct information from your provider, we can help you understand your rights, what to expect and how you can best utilise your package to get the very best use out of it.

FALLS AND BALANCE – if you have had a fall or are worried about your safety in your home we can conduct a full falls, balance and home safety audit

  • We can provide recommendations and costs for any equipment, aids or modifications (and arrange for it to be put  in place if you need)
  • We can tailor an exercise program and provide regular physiotherapy-based input on retaining and regaining balance, strength, confidence and the ability to get yourself safely off the floor if it happens again.
